WHO is Sound Generations?

We are a comprehensive non-profit organization that serves older adults and people with disabilities in King County. Our mission is to partner with older adults to provide accessible and inclusive services so they can age their way.

We are currently seeking a non-clinical Social Worker to join our team at Lake City Senior Center in Seattle, WA.

WHAT will I do?

The Social Worker is dedicated to helping their clients access critical social services, resources, and other support to improve their overall wellbeing. Issues faced by clients are often related to housing, poverty, unemployment, mental health, and self-neglect/ personal safety. The Social Worker will respond to client concerns, provides comprehensive assessments, and develops interventions that increase client stability, support independence, and prevent hospitalizations.

Pay Rate: $34.84/Hour

Work Schedule: Monday Through Friday, 40 hours per week. (Mondays, Wednesdays, and Fridays are spent on-site at Lake City Senior Center from 8:30am to 3:30pm. Tuesdays and Thursdays vary depending on whether any home visits are scheduled, but are largely work-remote days.)

Responsibilities:

  • Initiate home visits with client.
  • Engage client to identify needs through in-depth assessment utilizing evidence-based tools, considering cognitive, behavioral, social, and functional aspects.
  • Investigate complex psycho-social situations with collateral contacts, environmental observations, and when possible, review of prior attempts to engage client.
  • Develop and implement strengths-based care plan with client.
  • Mobilize clients' families and support circles to respond to client needs.
  • Facilitate referrals to community programs, advocating for needed services and follow up.
  • Seek and integrate cultural guidance from diverse community members/agencies about how to best serve individual clients.
  • Enact crisis intervention strategies and refer to health care providers, APS, DCR’s, and/or law enforcement. Report suspicions of abuse, neglect, exploitation, or fraud as a Mandatory Reporter to appropriate state or local authority and notify supervisor. Comply with other related laws, organization policies, and program procedures.
  • Advance care plans, track progress, and close contact when interventions are substantively accomplished.
  • Participate in team meetings to review assessments and interventions in case conference, to further strategize, collaborate, and confer with multidisciplinary team.
  • Ensure areas of responsibility operate within the policies and procedures of Sound Generations and applicable governing agencies.
  • Assist Supervisor with program planning, operations development, and community outreach.
  • Provide observations related to programmatic development to Supervisor.

Requirements:

  • Master's degree in social work, psychology, counseling, or related field.
  • WA State Department of Health clinical social work licensure (LICSW) or mental health counseling licensure (LMHC). Will consider those working toward licensure.
  • Experience performing psychosocial, psychiatric, and substance use assessments, and developing interventions relevant to risk, with older adult population.
  • Minimum three (3) years of professional experience working with older adults and/or individuals with cognitive or behavior issues, or comparable human services experience.
  • Geriatric Mental Health Certification or the ability to obtain certification within 18 months of hire “with CEU assistance through Sound Generations”.
  • Bilingual in Mandarin or Spanish preferred.
