Summary:
As a member of the MFP Team for the South Central Connecticut Area, the housing coordinator will provide housing coordination services to identified Money Follows the Person clients.
Essential duties and responsibilities include the following. Other duties may be assigned:
1. Identify potential units, including but not limited to assisted living units, apartments, individual homes, and subsidized housing based on the results of the client’s needs assessment and which meet the criteria of the MFP operating protocol.
2. Show and/or provide each client with at least two housing listings, except clients who found housing earlier.
3. Provide suburban listings first or exclusively to clients interested in relocating to suburban communities.
4. Assist clients with arranging transportation to view units and provide them with contact information for public or private transportation entities that can transport the participant from the nursing home to the unit.
5. Inform all clients that they are responsible for final transportation arrangements.
6. If the client's funds are insufficient to cover the cost, make them available within the limitations established in the budget for the transportation payment. Contact the transportation entity on behalf of clients with communication disorders.
7. Identify the need for and assist with the application for the Department of Social Services Rental Assistance Program (RAP) and Security Deposit Guarantee Program.
8. Work with Transition Coordinators to assess clients' housing needs, including identifying the need for modifications to a rental unit, based on the Department-approved accessibility checklist.
9. Identify and gain landlord approval regarding accessibility modifications required to make housing accessible to clients.
10. Coordinate accessibility modifications with the Department-approved contractor.
11. Provide required RAP briefings to each client applying for rental assistance at the client’s nursing home residence before transition.
12. Work cooperatively with Transition Coordinators to ensure a seamless transition from institutional to community living.
Education and/or Experience:
Bachelor’s degree from an accredited four-year college or university. Three years of direct experience in real estate may be substituted for a degree. Experience and/or knowledge of housing resources and landlord/tenant CT regulations is required.
Compensation details: 21-23 Hourly Wage
